Palliative Care Team

St Peters Surgery liaises extremely close with the palliative care team based at the pilgrims hospice in Margate to improve patient care and communication. Multidisciplinary Meetings (MDM) are held once a month between the GPs, District Nurses and the Community Hospice Nurses. The hospice team can arrange extra support for people with terminal illnesses. For more information please visit www.pilgrimshospices.org.

 

Social Services/Safeguarding

The surgery holds a risk stratification meeting every 6 weeks. We liaise closely and meet with Social Services to improve patient care and access to services to provide help to all of our patients that require extra support and care.

 

Adult Mental Health

The surgery meets with the Community Psychiatric Nurse Specialist every 6 weeks to discuss any patients that need extra support.

 

Health Visitors

Our health visitors are responsible for monitoring the growth, development and health of children under the age of 5 years.   If you wish to contact the health visitors for advice please call 01843 255 300.

 

Community Nurses
