Care home residents are now aligned to a specific Primary Care Network Practice therefore any patients who are currently registered with the practice or any patients who move into a care home will need to register with the practice that is providing the service to the care home. 

The benefits for all patients within the care home being registered with the one practice is that

structured medication reviews and ward rounds are performed with the care home having access to the one practice for all their residents
